Michigan Medicaid Application Assistance

Please call 734-764-5182 at the scheduled start time of your appointment. A UHS staff member will help you with determination of Michigan Medicaid eligibility. If it looks as though you may be eligible they will help you with the Michigan Medicaid application form. If possible, please have the following available for your call.

• Your last 30 days of income including:

• Check stubs

• Interest or dividend statements

• Self-employment income

If you find that you are unable to attend please cancel your appointment as soon as possible so someone on the waitlist can take the spot.
